• This form is for proposed events only and does not guarantee approval.
• All events must be submitted at least two (2) weeks before the proposed event date.
• After submitting this proposal, you are responsible for coordinating directly with the Store Manager at your selected location to discuss scheduling, staffing, and final approval.
• An event is not considered scheduled until it has been approved by the Store Manager.
